Definition

First-pass resolution rate (also called first-pass yield) is the percentage of claims that are paid on the first submission without any rework. It reflects how many claims sail through cleanly versus how many need correction, appeal, or resubmission.

It is closely tied to clean-claim rate but focuses on the end result — getting paid the first time — rather than just passing edits at submission.

Why it matters for billing

Every claim that is not resolved on the first pass costs staff time and delays cash. A high first-pass rate means a lean, efficient operation; a low one means money is tied up in rework. It is one of the best single measures of billing efficiency.

First-Pass Resolution Rate — frequently asked questions

How is first-pass resolution rate different from clean-claim rate?

Clean-claim rate measures passing edits at submission; first-pass resolution measures actually getting paid on the first try without rework.

Why does it matter for cost?

Each claim needing rework adds staff time and delays payment, so a higher first-pass rate lowers the cost to collect.
