What CPT 90838 covers
CPT 90838 is an add-on code for 60 minutes of psychotherapy provided with an evaluation and management (E/M) service in the same visit. It must be reported together with a primary E/M code — it cannot be billed by itself.
The two services have to be separately documented: the E/M work and the psychotherapy time are distinct, and the therapy time excludes the time spent on the E/M. Missing that separation is the usual reason an add-on like 90838 is denied.
