CPT 90839 bills the first 60 minutes of psychotherapy for a crisis — used for urgent assessment and treatment of a patient in high distress or with a life-threatening or complex situation. The reportable time range is 30 to 74 minutes.
For crisis work beyond 74 minutes, the add-on 90840 covers each additional 30 minutes. Because 90839 is time-based and urgent, the note must document the crisis nature and the total time — and it should not be billed alongside a routine psychotherapy code for the same encounter.
