99204 is where new-patient visits start carrying real weight. Moderate MDM typically means a combination such as multiple stable problems plus prescription management, or an undiagnosed new problem with uncertain prognosis, together with a moderate amount of data reviewed.
The most common risk driver here is prescription drug management, which by itself reaches moderate risk. We make sure the medication decision is documented — started, changed, or continued with monitoring — and that the data element (labs, imaging, external records) is reflected, so the moderate level is defensible.
It fits a new patient with two or more stable chronic conditions, one chronic illness with a mild flare, or an acute illness with systemic symptoms — where prescription drug management or moderate data review is in play.
