What CPT 90833 covers
90833 is an add-on to an evaluation and management visit. The E/M portion (for example a medication check) is coded on its own by its own rules, and 90833 is added for the psychotherapy provided in the same encounter. The two services must be separately identifiable in the note.
Only the psychotherapy time counts toward 90833 — time spent on the E/M service is excluded and must not be double-counted. The therapy time supporting 90833 is roughly 30 minutes; the record should reflect the therapy start/stop or duration apart from the medical service.
It fits a psychiatrist or other prescriber who, during a medication visit, also delivers a distinct block of psychotherapy — two separate services, documented and timed separately.
