What CPT 90840 covers
90839 covers the first 30–74 minutes of crisis psychotherapy; 90840 is added for each further 30-minute increment. It is an add-on and cannot be reported without 90839 for the same encounter.
Crisis codes require an urgent presentation with a life- or safety-threatening or highly distressing situation and the clinician’s full attention. Total crisis time must be documented so the number of 90840 units is supported by the record, counting only full additional increments.
It fits an acute crisis encounter — a patient in high distress or at risk — that runs well beyond an hour and requires continued crisis intervention.
