Please read these terms carefully before using EnVision MD Billing Services.

Welcome to EnVision MD Billing Services ("EnVision MD," "we," "us," or "our"), a medical billing and revenue cycle management company headquartered in Seattle, WA. These Terms and Conditions govern your access to and use of our services, website (envisionmdrcm.com), and any related platforms or communications.

1 Acceptance of Terms

By engaging EnVision MD Billing Services, accessing our website, signing a service agreement, or otherwise using our services, you ("Client," "you," or "your") agree to be bound by these Terms and Conditions and our Privacy Policy, which is incorporated herein by reference.

If you do not agree with any part of these terms, you must not use our services. These Terms constitute a legally binding agreement between you and EnVision MD Billing Services.

2 Description of Services

EnVision MD provides comprehensive medical billing and revenue cycle management (RCM) services to healthcare providers across the United States, including but not limited to:

  • Medical, physician, and hospital billing
  • Laboratory, radiology, and anesthesiology billing
  • Allergy, immunology, and emergency medicine billing
  • Mental health, behavioral health, and psychiatry billing
  • Medical coding and claim submission
  • Insurance verification and prior authorizations
  • Payment posting and accounts receivable follow-up
  • Provider credentialing services
  • Denial management and appeals

The specific scope of services provided to each Client will be outlined in a separate written Service Agreement. Services are subject to change with reasonable notice.

3 Eligibility & Client Responsibilities

Our services are available to licensed healthcare providers, practices, clinics, hospitals, and healthcare organizations operating legally within the United States. By using our services, you represent that:

  • You are a licensed healthcare professional or authorized representative of a healthcare organization
  • You have the legal authority to enter into this agreement
  • All patient and clinical information you provide to us is accurate, complete, and obtained with proper patient consent
  • You will comply with all applicable federal and state laws, including HIPAA
  • You will provide timely access to necessary patient data, insurance information, and documentation required to perform billing services
  • You will promptly notify EnVision MD of any changes to your practice, payer contracts, or credentialing status

4 Fees & Payment Terms

EnVision MD operates on a performance-based billing model — you only pay when you get paid.

  • Our fees start as low as 3.9% of collected revenue, with the exact rate determined by your specialty, volume, and service agreement
  • Fees are calculated on actual collections received, not on claims submitted
  • Invoices are issued monthly and are due within the timeframe specified in your Service Agreement
  • Late payments may be subject to a service charge as outlined in the Service Agreement
  • Credentialing and other one-time service fees, if applicable, will be disclosed in writing prior to commencement
EnVision MD reserves the right to adjust its fee schedule with a minimum of 30 days' written notice to the Client.

5 Confidentiality & HIPAA Compliance

EnVision MD acts as a Business Associate under HIPAA and is committed to safeguarding all Protected Health Information (PHI) we handle on your behalf.

  • A signed Business Associate Agreement (BAA) is required before any PHI is shared with EnVision MD
  • We use industry-standard encryption, secure systems, and access controls to protect all patient and practice data
  • We will never sell, lease, or disclose your confidential business or patient information to unauthorized third parties
  • All EnVision MD staff undergo HIPAA training and are bound by confidentiality obligations
  • In the event of a data breach, we will notify affected parties in accordance with HIPAA Breach Notification Rules

6 Intellectual Property

All content on envisionmdrcm.com, including text, graphics, logos, images, and software, is the exclusive property of EnVision MD Billing Services and is protected by U.S. copyright and intellectual property laws.

  • You may not reproduce, distribute, or create derivative works from our content without prior written permission
  • Our name, logo, and service marks may not be used without written authorization
  • Any billing templates, workflows, or proprietary processes developed by EnVision MD remain our intellectual property

7 Disclaimer of Warranties

Our services are provided on an "as-is" and "as-available" basis. While we strive for the highest standards:

  • We do not guarantee specific reimbursement amounts or claim approval rates, as these are ultimately determined by insurance payers
  • We are not responsible for payer policy changes, insurance denials outside our control, or regulatory changes affecting reimbursements
  • We do not provide legal, medical, or financial advice; our services are limited to billing and revenue cycle management
To the maximum extent permitted by law, EnVision MD disclaims all warranties, express or implied, including warranties of merchantability and fitness for a particular purpose.

8 Limitation of Liability

To the fullest extent permitted by applicable law, EnVision MD shall not be liable for any indirect, incidental, special, consequential, or punitive damages, including:

  • Loss of revenue or profits arising from claim denials by insurance payers
  • Interruptions in service due to circumstances beyond our control (force majeure)
  • Errors resulting from inaccurate or incomplete information provided by the Client
  • Third-party system failures, including payer portals or clearinghouse outages

In any case, our total liability shall not exceed the total fees paid by you to EnVision MD during the three (3) months immediately preceding the claim.

9 Termination

  • Either party may terminate with 30 days' written notice unless otherwise specified in the Service Agreement
  • EnVision MD may terminate immediately for cause, including non-payment, fraud, or violation of HIPAA or applicable law
  • Upon termination, all outstanding fees owed to EnVision MD become immediately due and payable
  • EnVision MD will return or securely destroy all Client data in accordance with HIPAA requirements and the BAA
  • Provisions relating to confidentiality, intellectual property, and limitation of liability survive termination

10 Governing Law

These Terms shall be governed by the laws of the State of Washington, without regard to its conflict of law principles. Any disputes shall be subject to the exclusive jurisdiction of the state and federal courts located in King County, Washington.

11 Changes to Terms

  • We will update the "Last Updated" date at the top of this page when changes are made
  • Active clients will be notified via email at least 14 days before material changes take effect
  • Continued use of our services after the effective date constitutes acceptance of the revised terms
